A clinical look at the FDA-cleared radiofrequency treatment most clinics oversell — and how it fits a real wellness plan, when hormones matter more, and who it's actually right for.

EMFEMME 360 has been one of the most marketed — and most overpromised — devices in women's wellness over the past few years. So before we tell you we offer it, we want to tell you what it does. And what it doesn't.

EMFEMME 360 is a non-invasive radiofrequency device made by BTL — the same company behind EMSCULPT NEO, EMSELLA, EmFace, and Exion. It's FDA-cleared, takes about 8 to 15 minutes per session, and requires no anesthesia, no incisions, and no downtime. Three sessions, one week apart, is the standard protocol.

We added EMFEMME 360 to our service menu at both Centennial and Alamosa because, used appropriately, it's a useful tool. Used as a standalone "fix" for symptoms that are actually hormonal in origin — which is how it's often marketed — it's the wrong answer.

This article is the long version of a conversation we have with patients several times a week. It's the difference between selling you a treatment and figuring out what's actually going on.

What EMFEMME 360 actually is.

EMFEMME 360 uses radiofrequency energy to deliver controlled heat to vaginal and vulvar tissue. The heat — described by most patients as warm and comfortable, similar to a hot stone — increases local blood circulation and stimulates collagen and elastin production in the treated area.

The mechanism is the same as other RF devices used elsewhere on the body (Exion, Morpheus8, Thermage). What makes EMFEMME 360 specific is the applicator design and the fact that it delivers 360-degree volumetric heating in a single short session. The device has both internal and external applicators, so it can address vaginal canal tissue and external vulvar tissue.

What the FDA cleared it for.

This is where most marketing gets sloppy. EMFEMME 360 is FDA-cleared for:

  • Temporary relief of muscle pain and muscle spasms
  • Temporary increase in local blood circulation
  • Heating for the purpose of elevating tissue temperature for selected medical conditions

It is not FDA-cleared for "vaginal rejuvenation," "vaginal tightening," "treatment of urinary incontinence," or "treatment of sexual dysfunction." Those are off-label applications — meaning the device may produce those outcomes through its mechanism, but the FDA hasn't formally cleared it to make those specific claims.

That distinction matters. It's why we describe outcomes as "many patients report improvements in" rather than "this treats." Honest framing is part of the treatment.

Based on what we see in our clinic and the broader clinical literature on RF energy in this anatomy, here's an honest summary:

  • Genuine GSM (genitourinary syndrome of menopause) — needs estrogen
  • Moderate to severe urinary incontinence — needs pelvic floor PT or specialist evaluation
  • Hormonally-driven low libido — needs hormonal workup
  • Vaginal pain due to true atrophy
  • Significant prolapse
  • Sexual dysfunction with relational or psychological roots

The treatment can produce noticeable improvement after a single session, with continued improvement over the following weeks. Most patients see their best results around 4 to 6 weeks after completing a series of three treatments. Results typically last 9 to 12 months, with maintenance sessions extending the benefit.

What we don't promise: dramatic transformation. What we look for: meaningful improvement in the specific symptoms a patient walked in with, paired with whatever else they actually need.

Who EMFEMME 360 is actually right for.

The patients who get the best results from EMFEMME 360 tend to share a few characteristics:

  • Their symptoms are mild to moderate. A patient with vaginal laxity from a recent vaginal birth will often see real improvement. A patient with severe atrophy after a decade of postmenopausal estrogen loss will benefit much more from vaginal estrogen first.
  • Their hormonal status is reasonably optimized. If estrogen and testosterone are at functional levels, the local treatment lands on a healthier tissue baseline. RF energy doesn't replace what hormones do.
  • They want a non-pharmacological option. Some patients can't or won't use vaginal estrogen — breast cancer history, personal preference, or other factors. EMFEMME 360 offers a tool that's not hormonal.
  • They're realistic about what's possible. Patients who come in expecting "before and after" miracles are usually disappointed. Patients who come in looking for meaningful improvement in a specific symptom are usually pleased.

Who it's probably not the right first step for.

We will sometimes turn patients away from EMFEMME 360 — at least as the first intervention. That includes:

  • Patients with moderate-to-severe genitourinary syndrome of menopause whose symptoms would respond more directly to vaginal estrogen
  • Patients whose primary complaint is low libido without local symptoms — that's a hormonal and sometimes psychological evaluation, not a device
  • Patients with active urinary tract infections or pelvic infections (treat first, device later)
  • Patients with significant pelvic organ prolapse (specialist referral)
  • Patients pregnant or recently postpartum (we wait until 3 months postpartum minimum)

The honest version

If you've been told for years that vaginal dryness or painful intercourse is "just part of menopause," that's wrong. There are several effective treatments. EMFEMME 360 is one of them. Vaginal estrogen is another. Systemic BHRT is another. Pelvic floor PT is another. The right answer depends on your specific symptoms, your hormonal status, and your preferences — which is why we lead with an evaluation, not a treatment.

How EMFEMME 360 fits a real wellness plan.

Most clinics that offer EMFEMME 360 are aesthetics-only. They sell the device. They don't have hormone clinicians on staff. They can't tell you whether your symptoms are RF-responsive, hormone-responsive, or something else.

That's the gap we're set up to close. A typical evaluation at Defiance Health for someone considering EMFEMME 360 includes:

1. The conversation first.

A 60-75 minute appointment — not a 15-minute device consult. We want to know what brought you in, what's changed, what you've tried, and what you're hoping for. Most of the diagnostic work happens here, not in labs.

2. Hormonal evaluation.

For most patients considering EMFEMME 360, a hormone panel is part of the workup. Estradiol, testosterone (total and free), progesterone, FSH, DHEA-S, thyroid panel, fasting metabolic markers. The panel tells us whether the underlying hormonal environment will support good outcomes — or whether we need to address that first.

3. Treatment plan, not treatment menu.

Based on the conversation and the labs, we map a plan. Sometimes that's EMFEMME 360 alone. Sometimes it's vaginal estrogen first, then EMFEMME 360 once tissue quality improves. Sometimes it's systemic BHRT first because the patient is in perimenopause or postmenopause and the local issue is downstream of the systemic one. Sometimes it's a referral for pelvic floor PT or to a urogynecologist.

4. Pelvic Power: EMFEMME 360 + EMSELLA.

For patients with both vaginal laxity concerns and stress urinary incontinence, BTL designed a combined protocol called Pelvic Power that pairs EMFEMME 360 with EMSELLA — the HIFEM (high-intensity focused electromagnetic) chair that strengthens pelvic floor muscles. EMFEMME addresses tissue quality and circulation. EMSELLA addresses the muscle layer underneath. They complement each other in patients with overlapping concerns.

5. Honest follow-up.

We track what's actually working. If a series doesn't deliver the improvement we hoped for, we say so and adjust. The goal is your outcome, not the appointment.

What an EMFEMME 360 session is like.

A typical session takes 8 to 15 minutes once you're in the treatment room. We use both internal and external applicators depending on what the treatment plan calls for.

Most patients describe the sensation as a warm, comfortable heat — frequently compared to a hot stone massage. There is no anesthesia required. Mild redness or swelling immediately after treatment is normal and resolves within a few hours. There is no downtime; you can resume normal activities — including exercise and intercourse — immediately.

The comprehensive intake — which is required before any device treatment — is $495 and includes lab work, a 60 to 75 minute provider visit, and a personalized treatment plan. The intake fee is flat and does not apply toward treatment cost. We charge for our clinical time, not for selling you a procedure.

Does EMFEMME 360 hurt?

The vast majority of patients describe the sensation as warm and comfortable — frequently compared to a hot stone massage. There is no anesthesia required. Some patients experience mild warmth that's slightly more noticeable in the most sensitive areas, but it should never be painful. Tell your provider during the session if anything feels uncomfortable so they can adjust.

How long do results last?

Hormonal status, age, and lifestyle factors influence longevity — patients with optimized hormones and healthy tissue tend to maintain results longer.

Is there any downtime?

None. You may notice mild redness or swelling immediately after treatment, but it resolves within a few hours. You can resume all normal activities — including exercise and intercourse — the same day.

How is EMFEMME 360 different from vaginal estrogen?

Vaginal estrogen replaces a missing hormone that supports tissue health. EMFEMME 360 stimulates blood flow and collagen production through radiofrequency energy. They work through completely different mechanisms and can be complementary. For genuine genitourinary syndrome of menopause (GSM), vaginal estrogen is usually the first-line treatment. EMFEMME 360 may add benefit on top of that — but it doesn't replace it.

Can I have EMFEMME 360 if I have a history of breast cancer?

Often yes, since EMFEMME 360 is non-hormonal. This is one of the populations the device may benefit most — patients who need a non-hormonal option for symptoms typically treated with vaginal estrogen. We always coordinate with your oncologist before treatment in cancer survivors.

How is EMFEMME 360 different from the O-Shot?

The O-Shot is a platelet-rich plasma (PRP) injection. It uses your own blood plasma to stimulate tissue regeneration. EMFEMME 360 uses radiofrequency heat. Different mechanisms, different evidence base. We don't currently offer the O-Shot — it has weaker clinical data than EMFEMME 360 and a more invasive delivery.

Will EMFEMME 360 fix urinary incontinence?

For mild stress urinary incontinence, EMFEMME 360 can help — especially when combined with EMSELLA (the HIFEM pelvic floor chair). For moderate or severe incontinence, or urge incontinence, you'll need a more comprehensive evaluation, which may include pelvic floor physical therapy or a urogynecology referral. We assess this during the comprehensive intake.

Can I do EMFEMME 360 while postpartum?

We typically wait at least 3 months postpartum before treatment, and longer if you're still breastfeeding or actively healing from significant tearing or surgery. The early postpartum period is one of natural healing, and we don't want to interfere with that process. EMFEMME 360 can be very effective for postpartum laxity once the body has had time to recover.

Do you offer EMFEMME 360 at the Alamosa location?

Yes. EMFEMME 360 is available at both our Centennial and Alamosa locations. The treatment protocol and pricing are the same at both clinics.

Is EMFEMME 360 HSA/FSA eligible?

It depends on your plan and whether the treatment is being used for a documented medical condition (like stress urinary incontinence) versus a cosmetic concern. We can provide documentation that supports HSA/FSA reimbursement when appropriate. We also accept Cherry and CareCredit financing.

Will EMFEMME 360 improve my sex life?

It depends on what's affecting your sex life. If the issue is local — laxity, dryness, reduced sensation due to blood flow changes — EMFEMME 360 can help. If the issue is hormonal (low libido from declining testosterone, for instance), or relational, or psychological, EMFEMME 360 won't address it.
