Your kid needs a physical to play this season. The school sent home the form. We'll handle the rest — properly, in 15-20 minutes, by a real clinician who actually evaluates your athlete instead of just signing a paper.

Defiance Health offers $45 sports physicals at our Alamosa location, built for the families across the San Luis Valley who need a clean, fast, professional clearance for school athletics. We see student athletes from Alamosa, Center, Monte Vista, Conejos County, and the surrounding communities.

We'll review the medical history, complete the cardiovascular exam, perform the musculoskeletal evaluation, do the vision screen, and sign the official CHSAA-compliant Pre-Participation Physical Evaluation (PPE) form. Your athlete leaves cleared to play.

What the visit includes.

The full PPE Examination

  • Medical history review (parent-completed form, reviewed in detail)
  • Vital signs — height, weight, blood pressure, pulse
  • Cardiovascular exam, including auscultation for murmurs
  • Pulmonary exam
  • Abdominal exam
  • Musculoskeletal evaluation (range of motion, flexibility, joint stability)
  • Neurological screen
  • Vision screen (Snellen)
  • Skin evaluation
  • Clinical clearance determination
  • Signed CHSAA-compliant PPE form
  • Brief conversation with athlete and parent if anything needs follow-up

What's not included: a full annual physical, immunizations, lab work, or treatment for any condition we identify. If we find something during the exam that needs attention, we'll tell you what's appropriate next — but the sports physical is its own focused service.

Why have us do it instead of an urgent care.

Most sports physicals in the San Luis Valley happen at urgent care, or via traveling sports physical clinics that come through schools once a year. Both work, but neither offers a particularly thorough exam. Patients are stacked, the clinician you see is whoever's on shift, and the evaluation is checkbox-paced.

We do them differently:

  • 15-20 minutes per athlete — enough time to do a real exam
  • Same provider every time for families who come back annually
  • Real conversation with both athlete and parent — sleep, nutrition, conditioning, prior injuries
  • Direct feedback on anything we notice that warrants follow-up — not just a signed form

The $45 price is intentional. We're not trying to undercut the cheap chains. We're trying to be the clinic the family chooses every year because the experience is better and the evaluation is real.

For the parent of the athlete

If your kid has a heart murmur nobody's mentioned, a flexibility issue that's setting them up for an ACL tear, or symptoms they haven't told you about — those are the things a real exam catches. We do the visit slowly enough to actually catch them.

Timing for each season.

We do sports physicals year-round, but here's when most families need them:

Fall sports — book July or early August

Football, cross country, volleyball, soccer, cheer, golf. Practices typically begin first week of August. Aim to have the physical completed by July 31 to avoid the rush.

Winter sports — book early November

Basketball, wrestling, swimming, ice hockey. If your athlete had a fall sports physical that's still within 365 days of the start of the winter season, no new physical is needed.

Spring sports — book early February

Track and field, baseball, softball, tennis, lacrosse. Same 365-day rule applies.

Colorado requires a sports physical to be valid for 365 days from the date of exam. If your athlete plays multiple seasons in the same school year, one physical typically covers all of them — as long as it's still within the 365-day window when each season starts.

What to bring to the appointment.

  • Your school's PPE form — most Colorado schools use the standard CHSAA form, which is what we sign. If your school requires a different form, bring that one.
  • Completed medical history section — this is the parent-and-athlete-completed portion. Filling it out at home saves us 5-10 minutes.
  • List of medications the athlete is currently taking, including over-the-counter and supplements.
  • Glasses or contacts if your athlete wears them — we'll check vision with corrected lenses on.
  • Photo ID for the parent at minimum (athletes under 18 need a guardian present for the visit).
  • Payment — $45 cash, card, HSA, or FSA at the time of visit.

Sports Physical FAQs.

Is the $45 the only cost?

Yes. $45 flat for the visit, the exam, and the signed form. No facility fees, no add-ons, no surprises. The only thing we'd bill separately is if your athlete needs follow-up care for something we identify — and we'd tell you the cost of that before we did anything.

Does my child need to be a Defiance patient?

No. Sports physicals are open to anyone — first-time visitors welcome. Bring the form and we'll handle the rest.

Do you accept walk-ins?

Yes, especially during peak pre-season weeks (last two weeks of July, first week of August, first week of November, first week of February). Wait time is usually 15-30 minutes during walk-in. Outside of peak season, we recommend booking online to guarantee a slot.

Does my child need to be present, or can I just bring the form?

The athlete needs to be present — this is a real clinical exam, not just a signature service. The provider needs to evaluate the athlete to clear them to play.

How long is the physical valid?

Sports physicals are valid for 365 days from the date of exam under Colorado regulations. If your athlete plays multiple seasons within a school year, one physical typically covers all of them as long as you stay within that 365-day window.

What schools do you serve?

Any school. We see athletes from Alamosa, Center, Monte Vista, Sangre de Cristo, Centauri, Sierra Grande, Sargent, Conejos County districts, and visiting students from out of district. The CHSAA-compliant form we sign is accepted by every Colorado public and most private schools.

Can you sign forms for non-school athletics?

Yes. We can sign physicals for club sports, USA Wrestling, AAU basketball, summer leagues, recreation departments, and most non-school athletic organizations. If there's a specific form, bring it. If your organization just needs general clearance documentation, we can write that.

What if you find something during the exam that's a concern?

We'll tell you immediately, document it on the form (or hold the form pending follow-up if needed), and discuss what next steps are appropriate. Most of the time, we're talking about something minor that doesn't disqualify your athlete — just something worth keeping an eye on. Occasionally, we'll need a more detailed evaluation before clearing them to play. If that happens, we'll explain why clearly.

Is the sports physical fee HSA/FSA eligible?

Yes. Sports physicals are a qualified medical expense and can be paid with HSA or FSA funds. We'll provide a receipt suitable for reimbursement claims.

What if my athlete plays multiple sports?

One physical covers all of them as long as it's within the 365-day validity window when each season starts. Most athletes only need one physical per school year.

Do you do physicals for college athletes?

Yes, but college athletic programs typically have their own required forms and protocols (NCAA, NJCAA, NAIA each have specific requirements). Bring whatever form your athletic department provides and we'll complete it. The cost is still $45 unless the form requires significantly more than the standard PPE evaluation.

What about my younger kids — middle school athletics?

Same physical, same $45 price. Middle school athletes often have less stringent forms required by their schools, but we do the full PPE evaluation regardless. Better to know now if there's something that should affect their athletic participation as they progress.
