Thyroid Optimization in Denver & Alamosa, CO
Most conventional thyroid panels only check TSH — and miss the full picture. At Defiance Health, we run comprehensive thyroid panels including Free T3, Free T4, Reverse T3, and thyroid antibodies, so we can identify dysfunction that standard labs routinely overlook.
Whether you have Hashimoto's thyroiditis, hypothyroidism, subclinical thyroid disease, or are simply exhausted and gaining weight with a "normal" TSH, our providers will dig deeper to find the root cause and build a treatment plan around you — not around a number on a lab report.
What We Test
Hypothyroid Symptoms Are Often Dismissed
Thyroid dysfunction is one of the most underdiagnosed conditions in medicine. Millions of people — especially women — are told their labs are "normal" while continuing to struggle with symptoms that are anything but normal. If any of the following sound familiar, a full thyroid panel may be the missing piece.
Thyroid Conditions We Diagnose & Manage
The thyroid is a small butterfly-shaped gland in your neck that controls nearly every system in your body — metabolism, energy, mood, heart rate, temperature regulation, and more. When it underperforms, even slightly, the downstream effects are profound.
Hypothyroidism
The thyroid doesn't produce enough hormone to meet the body's needs. Classic symptoms include fatigue, weight gain, cold intolerance, and brain fog. Often undertreated when only TSH is checked.
- Primary hypothyroidism
- Secondary hypothyroidism
- Subclinical hypothyroidism
- Post-thyroidectomy management
Hashimoto's Thyroiditis
An autoimmune condition where the immune system attacks thyroid tissue, causing fluctuating and ultimately declining thyroid function. Often missed because TSH can appear normal early on.
- TPO & thyroglobulin antibody testing
- Anti-inflammatory dietary support
- Gut health & immune optimization
- Hormone stabilization
Reverse T3 Dominance
Your body converts T4 into Reverse T3 (an inactive form) instead of active Free T3 — causing hypothyroid symptoms even when TSH looks fine. This is almost never tested in conventional care.
- Triggered by chronic stress or illness
- Calorie restriction & crash diets
- Iron deficiency & nutrient depletion
- Addressed with T3 therapy when appropriate
Subclinical Hypothyroidism
TSH is elevated but T3 and T4 are still in range. Conventional medicine often takes a "watch and wait" approach — but patients are frequently symptomatic and benefit from treatment.
- TSH between 2.5–10 mIU/L
- Symptom-based treatment decisions
- Cardiovascular & cholesterol implications
- Fertility considerations
Poor T4-to-T3 Conversion
Many patients on levothyroxine (T4-only) still feel terrible because their bodies don't convert it efficiently to active T3. Combination T3/T4 therapy or desiccated thyroid can make a dramatic difference.
- Liver & gut conversion factors
- Selenium & zinc deficiency
- Combination levothyroxine + liothyronine
- Natural desiccated thyroid (NDT)
Thyroid & Hormone Overlap
Thyroid dysfunction rarely occurs in isolation. Low estrogen, low progesterone, low testosterone, and adrenal dysfunction all impact thyroid function — and vice versa. We look at the whole picture.
- PCOS & thyroid connection
- Menopause & thyroid changes
- Low T & thyroid in men
- Adrenal fatigue & HPA axis
A Normal TSH Doesn't Mean a Healthy Thyroid
TSH (Thyroid Stimulating Hormone) is the only marker checked by most PCPs. It's a pituitary signal — not a direct measure of how much active thyroid hormone is reaching your cells. You can have a perfectly "normal" TSH and still have:
- → Low Free T3 — the only hormone your cells actually use
- → Elevated Reverse T3 blocking your T3 receptors
- → Positive TPO antibodies indicating Hashimoto's
- → Poor T4-to-T3 conversion due to nutritional deficiencies
At Defiance Health, we use functional reference ranges — not just the lab's broad "normal" ranges — and we treat the patient in front of us, not just their numbers.
Functional Reference Ranges
We target optimal Free T3 levels (upper third of the range), not just "not low." There's a big difference between barely normal and thriving.
Symptom-Driven Decisions
Numbers inform — they don't dictate. If you have classic hypothyroid symptoms and borderline labs, that matters. We treat the whole person.
Direct-Pay, No Gatekeeping
No referral required. We can order a full thyroid panel, review your results, and build a treatment plan in a single appointment.
Comprehensive Thyroid Lab Panels
We don't order one marker and call it a day. Our thyroid panels give us a complete picture of how your thyroid is functioning — from pituitary signaling all the way to cellular delivery. We can also bundle thyroid testing with a full hormone panel for a complete hormonal picture.
Core Thyroid Panel
Our baseline thyroid panel goes beyond TSH to measure actual circulating hormone levels. Free T3 is the active form your cells use — and the one most commonly missed.
Hashimoto's / Autoimmune Panel
If Hashimoto's is suspected, we add TPO and thyroglobulin antibodies. Positive antibodies confirm autoimmune thyroid disease — even when TSH appears normal.
Full Hormone + Thyroid Panel
Thyroid function is deeply connected to sex hormones and adrenal output. This bundled panel uncovers the full hormonal picture — ideal for anyone with multiple symptoms.
Nutrient & Conversion Factors
Selenium, zinc, iron, and Vitamin D are all required for proper thyroid hormone production and T4-to-T3 conversion. Deficiencies are extremely common and frequently overlooked.
Thyroid Treatment Tailored to You
There is no one-size-fits-all approach to thyroid treatment. Some people do well on T4-only therapy. Others need combination T3/T4. Some benefit from natural desiccated thyroid. We find what works for your body — not just what's easiest to prescribe.
Levothyroxine (T4 Therapy)
The most commonly prescribed thyroid medication, providing the precursor hormone T4. Works well for many patients who convert T4 to T3 efficiently. We dose to target optimal Free T4 levels, not just TSH suppression.
Combination T3/T4 Therapy
Adding liothyronine (T3) alongside T4 can dramatically improve symptoms for patients who convert poorly. Many patients who felt "fine" on T4-only therapy report life-changing results with combination therapy.
Natural Desiccated Thyroid (NDT)
Derived from porcine thyroid glands, NDT contains both T4 and T3 in a natural ratio. An excellent option for patients who prefer a whole-gland approach or who haven't responded well to synthetic options.
Nutrient Optimization
Selenium, zinc, iron, Vitamin D, and B12 are all essential for thyroid hormone production and conversion. We identify and correct deficiencies through diet guidance and targeted supplementation or vitamin injections.
Anti-Inflammatory Support
For Hashimoto's patients, reducing systemic inflammation through diet and lifestyle is foundational. Our team can guide you through elimination protocols, gluten reduction, and gut health strategies that reduce antibody levels over time.
Hormone Balancing
Low estrogen, progesterone, and testosterone all worsen thyroid symptoms — and thyroid dysfunction worsens hormone imbalance. We address both simultaneously through our BHRT program for women and TRT for men.

Thyroid Health Connects to Everything
Thyroid dysfunction rarely exists in isolation. Many of our patients find that addressing thyroid function improves outcomes across multiple health areas — and vice versa. Here are the services most commonly combined with thyroid care.
Bioidentical Hormone Replacement (BHRT)
Low estrogen and progesterone worsen thyroid symptoms and can suppress thyroid function. BHRT and thyroid optimization are a powerful combination for women in perimenopause and menopause.
Testosterone Replacement Therapy (TRT)
Low testosterone in men often co-occurs with thyroid dysfunction. Both conditions cause fatigue, weight gain, and brain fog — and both respond to targeted hormone optimization.
Medical Weight Loss
Thyroid dysfunction is one of the most common causes of weight loss resistance. Optimizing thyroid function is often a critical first step — or complement — to GLP-1 weight loss therapy.
Chronic Fatigue & Low Energy
Thyroid dysfunction is among the top root causes of chronic fatigue. If you've been told your "labs are normal" but still can't stay awake, a full thyroid panel is where we start.
Direct-to-Consumer Lab Testing
Order your own comprehensive thyroid panel without a referral. Results are reviewed by one of our providers who can discuss findings and recommend next steps.
Nutrition Coaching
For Hashimoto's patients especially, diet plays a major role in managing antibody levels and inflammation. Our nutrition coaching includes thyroid-specific dietary protocols.
Thyroid FAQs
TSH is a pituitary hormone — it tells the thyroid to produce more hormone, but it doesn't tell you how much active T3 is actually reaching your cells. You can have a "normal" TSH and still have low Free T3, high Reverse T3, or Hashimoto's antibodies — all of which cause significant symptoms. Additionally, the conventional TSH "normal" range (0.5–4.5 mIU/L) is very wide. Many functional medicine practitioners and researchers believe that optimal TSH for symptom resolution is closer to 1.0–2.5 mIU/L. At Defiance Health, we use functional ranges and treat your symptoms, not just your TSH number.
Hypothyroidism is a condition where the thyroid doesn't produce enough hormone. Hashimoto's thyroiditis is the most common cause of hypothyroidism in the United States — it's an autoimmune disease where the immune system attacks and gradually destroys thyroid tissue. You can have Hashimoto's (confirmed by positive TPO antibodies) for years before your TSH becomes elevated. This is why antibody testing is so important, especially in women with thyroid-like symptoms and a family history of autoimmune disease. Treating Hashimoto's also involves addressing the underlying immune dysfunction, not just replacing thyroid hormone.
Yes — this is one of the most common situations we see. Levothyroxine is T4-only, and many people don't convert T4 to active T3 efficiently. If your Free T3 is low despite adequate levothyroxine, you're not getting the full benefit of the medication. We frequently help patients on T4-only therapy transition to combination T3/T4 therapy or natural desiccated thyroid (NDT), which contains both T4 and T3. We also check your Reverse T3, which can block T3 receptors and cause continued symptoms. Many patients report dramatic symptom improvement after these adjustments.
Free T3 is the active thyroid hormone that binds to cell receptors and drives metabolism, energy production, and cognitive function. Reverse T3 (RT3) is an inactive mirror-image molecule that your body produces when it converts T4 — but it occupies the same receptors as Free T3, blocking it from working. High Reverse T3 essentially turns off your thyroid function at the cellular level, even when blood T3 levels look acceptable. RT3 is commonly elevated in chronic stress, calorie restriction, illness, adrenal fatigue, and low iron. It's rarely tested in conventional care.
Yes. We offer telehealth thyroid consultations and management for patients in Colorado, Arizona, California, and Washington. We can direct you to a local lab for blood draws, review results virtually, and prescribe thyroid medications through telehealth. Follow-up appointments are easy to schedule online, and we make prescription refills and lab ordering simple — so geography is never a barrier to getting the thyroid care you need.
Most patients notice meaningful symptom improvement within 4 to 8 weeks of starting or adjusting thyroid therapy. Energy and mood typically improve first. Weight, hair, and skin changes take longer — often 3 to 6 months. We retest labs at 6 to 8 weeks to confirm your Free T3 and TSH are optimizing, then again at 3 to 6 months once you're stable. It's a process of fine-tuning, but most patients feel substantially better within the first 2 to 3 months of optimal treatment.
No referral is needed at Defiance Health. We operate on a direct-pay model, which means you pay us directly — no insurance gatekeeping. Lab costs are typically billed separately through your lab of choice. We can provide superbills for you to submit to your insurance or HSA/FSA for potential reimbursement. Many patients find that the ability to order a full panel without fighting for insurance approval saves them years of frustration and misdiagnosis.
Absolutely. The thyroid regulates your basal metabolic rate — the calories you burn at rest. When thyroid function is low, metabolism slows significantly. Patients often gain 10 to 30 pounds without any dietary changes, and struggle to lose weight even with strict diet and exercise. This isn't a willpower problem; it's a hormonal one. Optimizing thyroid function — along with other metabolic factors like insulin sensitivity and sex hormones — is foundational to successful, sustainable weight management. We often combine thyroid optimization with our medical weight loss program for best results.
