What to Expect at Your First Hormone Therapy Appointment

Your first hormone therapy appointment is an information-gathering visit — not a commitment. Here's exactly what happens, how to prepare, and why there's nothing to be nervous about.

If you've been dealing with fatigue, brain fog, weight gain, mood swings, low libido, or sleep problems — and you're finally considering hormone therapy — the first appointment can feel like a big step. You might be wondering what they'll ask you, whether you'll need to get undressed, or if they'll start you on medication right away.

The answer to the last question is no. Your first hormone therapy appointment at Defiance Health is a conversation. It's where we learn about your symptoms, your history, and your goals — and where you learn about your options. No pressure, no commitment, and no medication decisions until after we have your lab results.

Here's a step-by-step walkthrough of the entire experience so you know exactly what to expect.

How to Prepare for Your First Visit

A little preparation goes a long way. The more information you bring, the more personalized your consultation will be. That said, if you forget something or don't have all the answers — that's completely fine. We'll work with what you have.

What to Bring to Your Appointment

Photo ID and insurance card (if applicable)
List of current medications (prescription, OTC, and supplements)
List of current symptoms and when they started
Relevant medical history (past surgeries, conditions, family history)
Any previous lab results (if you've had hormone levels tested before)
Questions you want answered (write them down so you don't forget)

Fasting tip: If labs are being drawn at your first visit (which they usually are), you may be asked to fast for 8-12 hours beforehand. Confirm when you schedule your appointment. Stay hydrated — water is always fine.

Wear something comfortable with sleeves that roll up easily (for the blood draw). There's no specific dress code and no need to overthink it.

Your First Appointment: Step by Step

1

Check-In & Intake Forms (15-20 min)

Arrive about 15 minutes early to complete your new patient paperwork — medical history, symptom questionnaire, consent forms, and HIPAA privacy notice. This gives your provider context before you sit down together. If you prefer to fill these out in advance, ask about digital intake forms when you schedule.

2

Vitals & Baseline Measurements (5 min)

A quick check of your blood pressure, heart rate, temperature, height, and weight. This is standard for any medical visit and gives us important baseline data.

3

One-on-One with Your Provider (30-40 min)

This is the heart of the appointment. Jessica Lara, PA-C will sit down with you and go through your symptoms, health history, lifestyle, and goals in detail. This isn't a rushed 5-minute visit — we take the time to understand your full picture. Expect questions about your energy, sleep, mood, libido, weight, stress levels, menstrual history (if applicable), and what you've tried before. Be honest — your provider has heard it all, and accurate information leads to better treatment.

4

Discussion of Options (10-15 min)

Once your provider understands your situation, they'll explain what bioidentical hormone therapy (BHRT) can do for your specific symptoms, how it works, and what delivery methods are available — such as pellets, injections, or topical creams. They'll be upfront about what hormone therapy can and can't do, how long it typically takes to feel results, and what the monitoring process looks like. This is also when you'll discuss cost, payment options, and whether your treatment may be HSA/FSA eligible.

5

Lab Work (10-15 min)

In most cases, we'll draw blood at this visit to get a comprehensive baseline of your hormone levels, thyroid function, metabolic markers, and other relevant labs. This is the data that drives your personalized treatment plan — we don't prescribe based on symptoms alone. The blood draw is quick (a few vials), and results are typically back within 5-7 business days.

6

Next Steps & Follow-Up Scheduling (5 min)

Before you leave, we'll schedule your follow-up visit — typically 1-2 weeks later once your lab results are in. That's the appointment where we review your labs together, finalize your treatment plan, and (if you're ready) begin therapy. You'll leave your first visit with clarity on the process, a timeline, and zero pressure to commit.

Total time: Plan for about 60-90 minutes for the full visit, including intake forms and lab draw.

What You Won't Have to Do

We know some people put off scheduling because they're anxious about what a first appointment involves. Here's what you can take off your worry list:

You will NOT:

✕ Be pressured into starting treatment that day

✕ Need a pelvic exam at your first visit

✕ Receive medication before your labs come back

✕ Be judged for your symptoms, your weight, or how long you waited to come in

✕ Be rushed through a 10-minute appointment

This visit is about gathering information — yours and ours. You're interviewing us just as much as we're assessing you. If at any point you feel like it's not the right fit, there's no obligation to continue.

What Happens After Your First Visit

After your first appointment, here's the typical timeline:

Days 1-7: Your lab results come back. Our team reviews them in detail before your follow-up.

Follow-up visit (week 1-2): You'll sit down with your provider to go through your results line by line. This is where we build your treatment plan — the specific hormones, the delivery method, the dosing, and the monitoring schedule. If you're ready, treatment can begin at this visit.

Weeks 2-8: If you start BHRT, most patients begin noticing improvements in energy, sleep, and mood within the first 2-4 weeks. Full optimization typically takes 8-12 weeks as dosing is fine-tuned based on follow-up labs and how you're feeling.

Ongoing: We monitor your hormone levels with regular lab work (typically every 3-6 months initially, then every 6-12 months once optimized) and adjust your plan as needed. You're never just put on a protocol and left alone — this is an ongoing partnership.

Common Concerns We Hear

"I've been dealing with this for years — will you take me seriously?"
Yes. Many of our patients come to us after being told their symptoms are "just stress" or "just aging" by other providers. We specialize in hormone health. If your labs show an imbalance, we treat it. Period.

"I don't know if I actually need hormone therapy."
That's exactly what the first appointment is for. We'll assess your symptoms and order labs. If hormone therapy isn't the right fit, we'll tell you that too. There's no incentive for us to recommend treatment you don't need.

"I'm nervous about side effects."
We get it. Every treatment carries some risk, and we'll be transparent about what to expect. Bioidentical hormones are designed to match your body's natural hormones, and we start at conservative doses, monitor closely, and adjust based on how you respond. You can also read our guide on managing treatment side effects for a sense of how we approach this.

"Is this safe? I've heard conflicting things."
There's a lot of outdated information out there — much of it based on the Women's Health Initiative (WHI) study from the early 2000s, which used synthetic hormones (not bioidentical). Modern BHRT has a strong safety profile when properly prescribed and monitored. We're happy to walk you through the research during your visit.

Frequently Asked Questions

How long does the first appointment take?

Plan for about 60-90 minutes total, including check-in paperwork, the consultation with your provider, and the blood draw. The consultation itself typically runs 30-40 minutes — enough time to cover your full history and answer all your questions.

Will I start medication at the first visit?

No. We never prescribe hormone therapy without lab results. Your first visit is for the consultation and blood work. We'll review your results together at a follow-up visit (typically 1-2 weeks later) and make treatment decisions then.

Do I need to fast before my appointment?

Usually yes — an 8-12 hour fast is recommended for accurate lab work. Water is fine (and encouraged). Confirm the fasting requirement when you schedule your appointment, as it may vary depending on which labs are being ordered.

Can I bring my partner or a family member?

Absolutely. Many patients find it helpful to have someone with them — whether for support, to help remember information, or because their partner wants to understand the process. They're welcome in the consultation.

How much does the first appointment cost?

Your initial consultation at Defiance Health is free. Lab work is a separate cost, and we'll give you a clear breakdown before anything is ordered. Many of our services are HSA/FSA eligible, and we offer financing through CareCredit and Cherry.

What labs will be ordered?

A comprehensive hormone panel typically includes testosterone (free and total), estradiol, progesterone, DHEA-S, thyroid (TSH, Free T3, Free T4), metabolic panel, CBC, vitamin D, and other markers based on your symptoms and history. Your provider will explain each test and why it's being ordered.

What if I'm not sure hormone therapy is right for me?

That's exactly why the first appointment exists. It's an information-gathering visit with zero obligation. You'll leave with a clear understanding of your options and your lab results — and the decision about whether to proceed is entirely yours.

Can I do a telehealth consultation instead of in-person?

The initial consultation can be done via telehealth if you prefer. However, the blood draw will need to be done in person at one of our locations — either Centennial (serving Denver and South Metro) or Alamosa (serving Southern Colorado).
